Hi Sarah,I would imagine the spots would be the same size on a medium. You're right,Lottie's is a large. She could probably have a medium but I've always ordered the large for her.
She also has the Purple Daisy one 
I refuse to put her in pink so she has red,yellow,orange and purple!
Niffnaff,I would trust the clips totally.Lottie isn't a puller as a rule but does tend to pull when she spots one of her many friends! Perhaps a member with a stronger,pully dog might reply for you just to reassure. I would have no hesitation in recommending Red Dingo Collars and Leads.

Oh Sarah, I'm sorry, I have such a rubbish memory. I think the spots were a tad smaller on the medium but can't be certain.
Sophie is tiny for a lab and I buy the large for her as they adjust loads, don't know if that helps any
