I'm being a bit thick here, but Cadbury had his nose job last week and I want to claim the money back.

The vet said bring in a claims form and they will sort it. But I don't seem to have a claims form.

I'm with PetPlan. Do I have to ring up and get a claims form or am I missing something obvious?

Hi, you can print a claim form off the petplan website-www.petplan.co.uk. They usually send one with your policy too? Just fill in your bits and sign it and take it to the vet and they will do the rest.

Yes print one off the website fill in your bits leave anything blank if you don't know hand to Vet and they do the rest and send it in then in a week or two you get a letter from PetPlan saying they are assessing your claim then about a week after that the cheque comes to the policy holder and you just pop it in the bank. simples! (If I can do it anyone can Wink ).

I have done several of these.

You often have to phone and specifically ask, and they will either send a form in the post or advise you on downloading one.

Usually you fill in about half of the form yourself, describing what the problem was, date of noticing it and of consulting the vet, and then the vet fills in the rest and posts it off, confirming what you have said, what treatment was given and what the cost was. You should then get everything back (minus your excess).

One tip though - do keep your end up with the vet. I have sometimes had to chase up a claim, and have found it delayed because they sat on it for a couple of weeks. Twisted Evil

we are with pet plan and have claimed quite a few hundreds of pounds over the years, Sad and have found them easy to deal with and they pay up pretty quickly, just fill your bit in and the vets will do the rest.

If you need to claim for long term medication such as metacam, rimadyl, plt etc, don't buy it from the Vet just get a prescription and buy the medication online. vetmedsdirect will send you an invoice which the insurers will pay out on. Why, you ask? If you claim less by buying your medication online your premiums don't go up as much next year or, you don't reach your claims limit as quick.
